Holistic Support
This service provides holistic support for those who are intellectually or physically disabled, and the older person. It provides holistic home-based support service for individuals and whanau to live independently in an environment that is supportive to their lifestyle and disability and promotes positive ageing for the older person.
Referral Process
Support for independent living receives a referral from the funding provider - Accessibility. If the client is eligible to access the service, they are then assigned a Key Worker and Support Team. From the needs assessment, the Key Worker will work closely with the client to achieve the outcomes and goals described in their assessment.
