Community & Home-based Care
Nga Oranga o te Rae community and home based care is provided to Tangata Whaiora and their whanau. This service provides thorough needs assessments, advocacy and support, education awareness, facilitation with whanau, referral to appropriate agencies, information sharing, integrated care facilitation and health promotion events.
- Nga Oranga o te Rae Child, Adolescent and Family community and home based care is provided to tamariki through to rangatahi and their whanau.
- Nga Oranga o te Rae Adult community provide support to Tangata Whaiora to build confidence and to repair culture dislocation within the community.
- Nga Oranga o te Rae clinical component is available to all Tangata Whaiora clients.
- Alcohol and Drug Support is offered to rangatahi, adults and the older person.
Referrals to Nga Oranga o te Rae can be made through self referrals, whanau, General Practitioners, Child Youth and Family, Restorative Justice Trust and other external agencies.

Additional to this is the Maori Youth Contestable fund which provides community and home-based care to youth at risk by referral from Child Youth and Family only.
